Your employer requires the vaccine, you put together a form that says sure, as long as the employer understands the vaccine has potential side effects up to and including death, and agrees to pay all medical expenses and provide a survivor benefit to dependents. Since THEY believe it’s safe and effective, they should happily agree. EACH employee walks in to their manager’s or HR reps office with the same form, and if the employer declines to accept liability, you now know what they really think of the vaccine they’re requiring.
