Vaccination Declination Waiver/Vaccine Rant/Mad Cow disease

Putting on the tinfoil here about a recent "Mandatory" waiver I had to sign that basically gave me a choice of whether I accepted or declined an Immunization at work. Nothing like this has ever been presented to me before and I have been in the workforce for 13-14 years. Now I get it training at the workplace isn't unusual especially for me, but when training is packaged with a follow up immunization waiver I get a little Paranoid, especially since I don't trust Vaccines in the first place. Please forgive me if this sounds overly paranoid, or misguided, I genuinely have never been presented a mandatory waiver like this.
